Accessing Your Medical Records

You have the right to view your personal medical records and there are a number ofways this could be achieved depending on what you require.

For day-to-day management we recommend setting yourself up for online services, giving you access to your records and a range of self-management tools such as prescription requests and test results.

If you require full details of all of/part of your medical record you will probably need to complete a Subject Access Request. Read on below for further information.

We highly recommend setting yourself up with online access if you want to take control of your own medical records. You can access your basic medical records including test results, allergies, immunisation record and prescription request services. In the future we will be able to open up access to much more including making appointments and hospital letters. Visit our Online Services Page to find out how.

If you require access to large quantities of information we will need make a "Subject Access Request" (SAR), an agreement that allows you to make a request for a specific date range or type of record and gives the surgery adequate time to gather the information required. To make a SAR please complete the form below and return to the Health Centre or contact a member of our admin team.
