Going abroad on holiday or business? You may need travel
vaccinations, depending on the country you intend to visit.

Telephone the Health Centre and ask for a travel
form for each person who will be going abroad. |

Complete the form(s) in full, and then make an appointment
with the Practice Nurse to discuss the requirements
for the area you will be visiting. |

Bring the form(s) with you to your appointment, and the nurse
will work out a programme for the necessary vaccinations. |
PLEASE ALLOW PLENTY OF TIME!!
Some vaccines need to be given 3 months in advance of
travel.
Some vaccines are not kept in stock as they are not often
requested. These can be ordered for you on a named basis
and kept for your use only.
You will be advised at the first consultation of any
costs involved. You may be asked to pay before the vaccines
are ordered.
All patients receiving injections and vaccinations are
advised to stay at the Health Centre for 20 minutes afterwards
- please allow time for this.

Vaccines are charged at cost price; they may be subject
to price increases, and an administration fee
will be added: single booster £10.00, £25.00 course
of vaccines.
