Prescription Requests
Regular repeat prescriptions are printed
on our computer and the sideslip provides a list of your
medicines and the date by which these should be reviewed
by you and a Doctor or Nurse at the Health Centre. Please
give the sideslip to the Health Centre, Moreton Pharmacy,
branch surgeries, send by post or you can fax on 01647
440089. Please allow 3 workings days for the Health Centre
to prepare your prescription and the chemist to have your
medicines available.
We do not take requests by telephone for repeat prescriptions
Alternatively
if you wish to order a repeat prescription by e-mail,
please complete this form and send it to
the Surgery. Please remember e-mails are not confidential.
