General information

The Health Centre is all on one level, with toilet facilities for wheelchair users.

Office Hours

Staff are available at the Health Centre from 8.30am - 12.45pm and 2.00pm - 6.00pm weekdays. Outside these hours there is a doctor on call at all times. The on-call doctor can be contacted by telephoning the Health Centre (01647 440591) where you will be given a number to ring.

When the surgery is closed

When the surgery is closed and you need a doctor, please ring the Emergency out of hours phone number 0845 6710270, so that a Doctor can be contacted for you. Please note for your safety the services mentioned record all calls.

Devon Doctors also now provide an out of hours dental service, on 01392 823682. This service is available 6.30pm to 8.00am weekdays, and Saturdays, Sundays and Bank Holidays 10.00am to 1.00pm. Please be aware that you should contact the above number before attending the clinic.

Alternatively NHS Direct which is a 24 hour telephone helpline staffed by nurses and trained operators may be contacted on 0845 4647.

Out of hours services are provided by Devon Primary Care Trust, Newcourt House, Old Rydon Lane, Exeter, EX2 7JU. Telephone no: 01392 449700. Details of Primary Medical Services in the area may also be obtained from the Primary Care Trust.

New Patients

To register, you will be asked to fill in a form at the Health Centre. You will need to provide some form of identification, (e.g. a passport or driving licence). When you have registered you will be invited for a new patient medical with the Practice Nurse or a doctor, which will include a general health check. Personal and medical information is kept on computer at the Health Centre. It is only shared with those who need it for medical reasons. Your demographic details (which consist of your name, address, date of birth and NHS number) are also on the National Care Records Service or "Spine". If you would like further information about this, please ask one of the receptionists or look at the website www.connectingforhealth.nhs.uk. The area covered by the practice is shown below.

If you need to receive your medical information in an alternative format (eg large print) or another language, please inform one of the staff.

NHS Numbers

Patients are advised to keep a record of their NHS number, perhaps along with details of their next of kin, or person to contact in case of emergency. If you unexpectedly need medical treatment, this could help in your care, and in ensuring that any necessary information was passed on to your GP.

If you do not know your NHS number, please ask at the Health Centre and the staff will be able to help you.

Suggestions and Complaints

Any suggestions for improving our service are very welcome.
Complaints may be addressed to the Practice Manager or one of the Doctors.

Research

The practice sometimes recruits patients for studies run by the Primary Care Research Network. All studies have been approved by the NHS Ethics Committee. If you are invited to participate in research there is no obligation to do so, and if you decline this will not affect your treatment in any way. Anonymised patient data may be used for research that is in the best interests of patients and the NHS as a whole. If you wish to opt out of data collection for the purposes of public health surveillance, please inform one of the GPs.

Training

Medical students spend time with us as part of their training. We hope you will not mind if they are sometimes present at your appointment. There is no obligation to agree to this, and if you do not wish a student to be present please tell one of the receptionists or the doctor. All students are bound by the same rules of confidentiality as the doctors and other staff.

Morecare - Healthcentre Befrienders Group

There is a group of local volunteers who will help patients living in the Health Centre catchment area. They will tackle such jobs as shopping, driving, dog walking, sitting, befriending etc. If you are interested either in becoming a befriender or are in need of help, please phone Morecare's volunteer coordinator on 0800 622 6245 between 8am & 6pm. You may get an answerphone, so please leave a message if necessary.

Friends of the Health Centre

The Friends of Moretonhampstead Health Centre was formed several years ago. They have raised funds through various donations and bequests and have purchased items of equipment for use in the Health Centre and the Community.
